Summer Camp Registration Forms:
• Registration Form (or you can register online)
• Enrollment Packet (must be completed by all in addition to registering online)
• Family Handbook
• Camp Change Form (if needed)
• Inclusion Services Form for New Participants (if needed)
• Inclusion Services Form for Returning Participants (if needed)
• Chronic Health Conditions Form (if needed)
• Medication Authorization Form (if needed)
Register Online: Reserve a spot and submit your payment online. If more forms are needed, you will be contacted after registration. If signing up for Fun Club Before & After Camp Care, that registration must be added separately. Online payments cannot process 3rd party payments or financial assistance.
Register at our Welcome Center: Complete the registration forms and visit the Welcome Center. There, you'll select your sessions, reserve a spot with a non-refundable deposit, 3rd party payments or financial assistance. Fun Club Before & After Camp Care must be registered separately.
